While the abnormalities that produce dependence appear to resolve after detoxification (within days or weeks after opioid use stops), the abnormalities that contribute to addiction are more wide-ranging, complex, and long-lasting.
Certain medications, such as methadone, LAAM, buprenorphine, and naltrexone, act on the same brain structures and processes as addictive opioids, but with protective or normalizing effects. Methadone treatment, for example, reduces relapse rates, facilitates behavioral therapy, and enables patients to focus on life tasks such as maintaining relationships and holding jobs.
Naltrexone, on the other hand, is used to help patients avoid relapse after they have been detoxified from opioid dependence. It works by monopolizing mu opioid receptors in the brain, preventing addictive opioids from linking up with them and stimulating the brain's reward system. While naltrexone clings to the mu opioid receptors 100 times more strongly than opioids, it does not promote the brain processes that produce feelings of pleasure.

An estimated 45% of people who use heroin started with an addiction to prescription opioids. This pattern of escalation is particularly alarming given the potency of drugs like fentanyl, a synthetic opioid that is 50 times more potent than heroin and 100 times more potent than morphine.
The dangerous opiate abuse side effects, coupled with the potential for rapid progression from prescription misuse to illicit drug use, underscores the critical need for early intervention and comprehensive treatment strategies. The opioid crisis is not just a public health issue, but a complex, multifaceted problem that requires coordinated efforts from healthcare providers, policymakers, and communities.
