For individuals aged 6 months and older who are moderately or severely immunocompromised and have not been vaccinated, the CDC recommends a regimen of 2 or 3 doses of an updated COVID-19 vaccine. Further doses may also be required. In some cases, a patient may receive a Moderna vaccine after receiving the first or second Pfizer-BioNTech vaccine.

It's important to note that non-live vaccines can generally be administered to immunocompromised individuals without an increased risk of vaccine-associated adverse events. However, the magnitude and duration of vaccine-induced immunity may be reduced. In contrast, live vaccines should generally not be given to severely immunocompromised individuals due to the risk of disease caused by the vaccine strains [4].

Immunization Guidelines

When it comes to immunization guidelines, the timing of vaccination is significant for immunocompromised individuals. Vaccinations should ideally be given to these individuals before the initiation of immunosuppressive therapy to ensure optimal immunogenicity. Non-live vaccines should be administered at least 14 days before the start of therapy, while live vaccines should be given at least 4 weeks prior to the therapy.

In some cases, immunoglobulin therapy may be indicated for temporary passive protection against some infections. For severely immunocompromised children, a monoclonal antibody preparation like Palivizumab might be considered to reduce complications of respiratory syncytial virus infection.

Adequate immune response to vaccines may not be achieved in immunocompromised individuals, and serologic testing may be considered to ascertain immune response and guide re-vaccination and post-exposure management. However, the results of serologic testing may be affected by passive antibody in those receiving immunoglobulin therapy [4].

A vaccination recovery plan was implemented to address missed vaccinations, and after one year, there was a noticeable recovery of missed vaccinations, especially among younger children and for primary cycles.

Despite these efforts, deficits in coverage for routine vaccinations persist globally. For instance, in 2021, the 36-month coverage rates for polio and measles-containing-vaccine were 92.5% and 93.7% respectively, indicating a decrease compared to 2020. By 8 years of age, the immunization coverage for these diseases decreased further. The coverage rates for Men B, Men ACW135Y/C, and PNC vaccine at 36 months were 56.6%, 73.2%, and 86.9% respectively, also a decrease compared to 2020. The coverage rate for HPV vaccination in 2021 was 44.2%, another decrease compared to 2020.

There was also a significant increase in vaccination coverage for Men ACW135Y/C [5].

However, the drop in vaccination coverage during the COVID-19 pandemic has had a significant impact on routine childhood immunizations. Efforts are needed to administer missed doses and improve routine immunization program capacity to prevent future disease outbreaks.
