This presentation explores the complex nature of trauma and its profound impact on mental, emotional, and physical well-being. Through a trauma-informed lens, it examines how traumatic experiences—ranging from acute events to chronic or generational adversity—affect brain function, particularly the limbic system and prefrontal cortex, and contribute to symptoms such as emotional dysregulation, dissociation, and hypervigilance. The presentation highlights the strong correlation between trauma and substance use disorders, underscoring how trauma can both precede and result from substance abuse. It introduces both traditional (e.g., CBT, EMDR, prolonged exposure therapy) and alternative (e.g., nature-based therapy, bilateral stimulation) treatment approaches, emphasizing the importance of holistic healing strategies. The concept of intergenerational trauma is also addressed through the emerging science of epigenetics. With an emphasis on education, empowerment, and breaking cycles of inherited pain, this presentation aims to deepen understanding and promote effective support for individuals navigating the lasting effects of trauma.
A. Describe the neurological impact of trauma on brain function and behavior, and identify how trauma affects key brain structures (e.g., amygdala, hippocampus, prefrontal cortex) and how these changes contribute to emotional dysregulation and survival-based responses.B. Recognize the relationship between trauma and substance use disorders.C. Utilize effective trauma-informed interventions and pathways to healing and describe at least three evidence-based or alternative therapeutic approaches used to process and heal trauma.

As our understanding matures regarding the dysfunctional neural circuitry that drives addiction, evidence-based approaches exist that further legitimize the concept of addiction being a medical disorder that involves the brain. Individuals struggling with co-occurring mental health conditions additionally have to navigate through unique challenges and vulnerabilities- which have only intensified with the pandemic and its aftermath.
Despite the hedonic, selfish choice that addiction may resemble when looking in from the outside, the goals for effective assessment and treatment are to provide clarity on diagnoses and appropriate interventions for individuals and family members/loved ones. The overarching end goal is to facilitate informed decisions that would support their ongoing long-term recovery.
The purpose of this presentation will be twofold, initially to review the components of a multidisciplinary comprehensive evaluation as an adjunct to or in lieu of a traditional intervention, ultimately with the goal of rendering an unbiased opinion with recommendations.
The nuances of providing intensive treatment for substance use disorder and co-occurring mental health conditions will then be explored, highlighting the importance of a robust continuing care plan post-treatment. This includes the challenges of navigating through interactions with professional regulatory entities that may be involved, legal hurdles and building a safe transition plan if the individual plans to return to work.
Dr. Anish John is the Medical Director at the Positive Sobriety Institute, Chicago IL where he oversees care for patients in the partial hospitalization and intensive outpatient programs. He is board-certified in General and Addiction Psychiatry (ABPN) and Addiction Medicine (ABPM) and has an adjunct faculty appointment as Assistant Professor at Northwestern University Feinberg School of Medicine, Department of Psychiatry and Behavioral Sciences.
Dr. John has an active interest in teaching and is involved in the ongoing training of Psychiatry residents, Addiction Psychiatry and Addiction Medicine fellows. Dr. John has research interests that include the interface between digital technology and addiction, biomarkers and neurobehavioral predictors for recovery, the assessment and treatment of impaired health care practitioners and utilizing neuro-modulation techniques to influence the outcome of psychiatric/addictive disorders.
